I've never had a problem with this plug-in and have used it on several sites. There was an issue today, when installing to a site running Thesis, which turned out to be the cause and not the plug-in. Long time i looked for a feedback plugin to be easy to use, and forward the notifications via email. This plugin fits almost all my needs.
